Subluxation of the cervical vertebra in a child: causes and treatment methods

Subluxation in the cervical spine is quite common. About a third of all damage to this segment of the column is due to this injury. However, due to the scarcity of clinical manifestations of subluxation of the cervical vertebra in a child, in most cases, does not attract the attention of parents and non-traumatological specialists. At the same time, an untreated subluxation, in the future, can cause quite serious diseases. And not only the cervical spine.Anatomical reference When describing this type of injury, we are usually talking about subluxation of the first vertebra (Atlanta). To be more precise, the displacement occurs in the joint between it and the second (axial - axial) vertebra. The atlas is very tightly connected to the occipital bone of the skull. Head movements (turns and rotations, less - tilting and tilting) are possible in a large volume thanks to the CI-CII joint.Axis has a process - tooth, around which the Atlantean rotates, having a ring-shaped shape. The joint is reinforced with many ligaments that are not strong enough in childhood.Causes of damage The richness of head movements has a downside. With sharp or excessive turns, especially in combination with bending or tilting, the articular surface CI may slightly slip off the tooth CII. This situation is called "rotational subluxation of the first cervical vertebra".Children can get such an injury when performing somersaults over their heads if the grouping is not done enough or incorrectly. Hitting the water when diving headfirst or on the bottom in shallow water, sports games, jokes with "unscrewing the head", fights are also a fairly common cause of rotational subluxation of Atlanta. This damage is very likely, in combination with others, in case of accidents on transport or falling from a height. At the same time, the height may be insignificant. Many children like to hang upside down on the horizontal bar, holding on, bent at the knees, legs. In babies, rotational subluxation can occur even when wearing a T-shirt (turtleneck) with a narrow throat over the head.Manifestations of subluxation The first sign of a possible injury to the cervical spine is localized pain on one side. In the same direction, the head occupies a forced position - turned and slightly tilted. You may notice swelling (puffiness). If you feel the neck muscles in this area, they will be tense, and the pain will increase. Pain syndrome will not allow you to give the head a normal (average) position, especially to turn it in the opposite direction. Not necessarily, but quite often, rotational subluxation can be accompanied by headache, dizziness, loss of consciousness, insomnia. This is due to some compression of the vertebral artery passing in the lateral channels of the vertebrae on both sides, as well as partial compression of the carotid artery due to edema on the side of the injury.Other neurological reactions may occur due to injury (and/or compression by edema) of nerve roots. This may be manifested by tinnitus, visual impairment, a feeling of numbness or pain in the arms, shoulder girdle, lower jaw.First aid and treatment for subluxation of the atlas In case of suspected injury of the cervical spine, like any other injury, the first thing to do is to immobilize. For these purposes, a strip of cardboard (several sheets of thick paper) with a width equal to the height of the neck and a length equal to the size of the shirt collar is quite suitable. It is better to wrap cardboard or paper with a soft cloth. The finished retainer is carefully placed on the neck so that the free ends are on the back surface (as the collar is put on). If the length of the strip turned out to be longer, its ends can be wound one on top of the other. A slight (up to 1 cm) divergence of the ends is also acceptable. You can fix the tire with a bandage or a thin scarf (handkerchief). An independent attempt to correct the subluxation is unacceptable! It is necessary to seek qualified traumatological help as soon as possible (district trauma center, hospital with a surgical profile department, ambulance team call).Specialized diagnostic methods (radiography, CT, MRI) make it possible to determine the localization and type of subluxation, the need for its correction (minor displacements in children are corrected independently after the convergence of edema, muscle relaxation and pain disappearance). One or another tire is necessarily applied (depending on the type of injury), dynamic control is assigned. In some cases, hospitalization is indicated.
